HIMS: Drug/Alcohol Evaluation

Required Records

  1. If the FAA has any records for you, then these will need to be requested (to be sent directly to the neuropsychologist). Dr. Hoyman will provide a form for you to complete to request these records.
  2. Copies of all records regarding prior psychiatric/substance-related hospitalizations, observations or treatment not previously submitted to the FAA.

 

Evaluation Process

In-person and includes a clinical interview, collateral interview with parent/guardian/etc., comprehensive neuropsychological testing, a computerized screener, and feedback session (if desired).
