ADHD: Standard Track Evaluation
ADHD: Standard Track Criteria
- The applicant has had treatment or use of ADHD medication (for any reason) within the past four (4) years; or
- The applicant has had any ADHD symptoms in the past four (4) years; or
- The applicant has had any instability in academic, occupational, or social functioning within the last four (4) years; or
- The applicant has had any other psychiatric condition(s) or diagnosis (current or historical)
Required Records

The FAA requires specific documents to be provided prior to the evaluation
- If the FAA has any records for you, then these will need to be requested (to be sent directly to the neuropsychologist). Dr. Hoyman will provide a form for you to complete to request these records
Evaluation Process
In-person and includes a clinical interview, collateral interview with parent/guardian, comprehensive neuropsychological testing, a computerized screener, and feedback session (if desired).
Urinalysis Requirements:
The applicant will need to complete a urinalysis for amphetamines AND methylphenidate within 24 hours after completing testing (i.e., the day after the evaluation). Neuropsychological testing data cannot be used without the supporting urinalysis and may result in deferral and/or request for re-evaluation. The urinalysis will be at your expense.
- Identify a local drug testing laboratory
- Confirm that the lab can run BOTH amphetamines AND methylphenidate (this is not the same thing as methamphetamine)
- Check hours of operation
- Check if walk-ins are taken for drug testing or if an appointment is required
- Coordinate with your AME or your PCP to order the laboratory tests
